O2 bar add on for Float Members

Only fill this out if you are starting a membership! This should only be done, in-person, at "The Float Place".

I would like to enroll in the "02 bar add on" Membership program at “The Float Place”. My membership is $10 per month, and I am able to use the O2 bar every time I come into float.

O2 bar credits are not shareable and will not roll over.

There is a 3-month billing cycle minimum. Credit/Debit Cards are kept on file and are charged each month on the same date.

There is a fee of $10 when you cancel/reschedule within 24 hours.

There is no cost to cancel the membership, however if you cancel prior to the 3-month minimum you will be charged full retail price for all O2 sessions used prior. This membership automatically cancels when you cancel your float membership, or if you choose to cancel.

To cancel, please call us at 631-627-6161 and email us at floatmembership@gmail.com and you will receive a return email within 24 hours verifying the cancellation.
