To help us prepare for your event, we ask that the party host complete this form at least 3 days prior to the event date.

Please upload:

A clear photo of the guest of honor for our Birthday TV Display (shoulders up, no other individuals visible).
A photo of the party host’s driver’s license. The name must match the card used for checkout.

All information submitted through this form is used solely for event verification and preparation purposes.
