For and in consideration of allowing me to enroll and participate in any Basketball and/or Softball games or events organized and sponsored by the Landau Lawyers League in the County of Los Angeles, I hereby forever voluntarily release, discharge, waive and relinquish any and all actions or causes of action for personal injury, property damage or wrongful death occurring to me arising out of the participation in said sport or any activities incidental thereto wherever or however the same may occur and I do for myself, my heirs, successors, executors, administrators, assigns and other representatives hereby release, waive, discharge and relinquish any action or causes of action, aforesaid, which may hereafter arise for myself and for my heirs, successors, executors, administrators, assigns and other representatives, all of whom shall not present or prosecute any claim for personal injury, property damage or wrongful death against the Landau Lawyers League, the Lawyers League Management Corporation, the Los Angeles Unified School District, The Board of Education of City of Los Angeles, and the City of Los Angeles Department of Recreation and Parks and/or any of the foregoing organizations’ officers, agents, employees, representatives or officials (hereinafter referred to as the “Released Parties”) for any of said causes of action whether the same shall arise by the negligence of any of said Released Parties or otherwise, to the fullest extent permitted by law. I agree to protect, defend, indemnify, save, and hold harmless the Released Parties from any and all liability, loss, damage, cost or expense arising from my negligence or reckless or willful misconduct in connection with my use or occupancy of Los Angeles Unified School District or City of Los Angeles premises and facilities and/or any other premises and facilities utilized by me in connection with the above activities.
I acknowledge, understand and assume the risks inherent in Basketball and/or Softball, and I understand the said sports entail risks to my person and property; and, I am participating with full knowledge of said risks.
Participation in and spectating Basketball and/or Softball includes possible exposure to and severe illness from communicable/infectious diseases, including but not limited to COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist; I freely assume all such risks related to illnesses, such as COVID-19. If I am sick, have tested positive for, have been exposed to, or am showing symptoms of COVID-19 within the last 14 days, I will not attend any LLL games or events. If infected, I cannot participate until I receive a negative COVID-19 test.
I acknowledge, understand and assume the risk, if any, arising from the conditions of the various basketball courts and softball fields and adjacent school or park grounds and parking lots and acknowledge and understand that included within the scope of said waiver and release is any action or causes of action arising from the performance, or failure to perform, maintenance, inspection, supervision, control or security of said areas and for the failure to warn of dangerous conditions existing on or near said locations or for any action or causes of action for negligent supervision or selection of officials, spectators, players or coaches.
IT IS MY INTENTION, BY THIS INSTRUMENT, TO EXEMPT AND RELIEVE THE RELEASED PARTIES FROM LIABILITY FOR PERSONAL INJURY, PROPERTY DAMAGE AND/OR WRONGFUL DEATH CAUSED BY NEGLIGENCE.
I, for myself, my heirs, successors, executors, administrators, assigns and other representatives agree that in the event any claim for my personal injury, property damage and/or wrongful death shall be prosecuted against the Released Parties I shall save, defend, indemnify and hold harmless the Released Parties from any and all claims or causes of action by whomever or wherever made or presented for my personal injuries, property damage and/or wrongful death.
I have read and hereby agree to abide by the LANDAU LAWYERS LEAGUE PLAYERS CODE OF CONDUCT. I further acknowledge that my participation in the LANDAU LAWYERS LEAGUE will be in jeopardy should I fail to adhere to this code.
I understand that only players fully vaccinated against COVID-19 are eligible to participate in any Landau Lawyers League games or events, and I certify that by electronically signing below that I am fully vaccinated against COVID-19 and a true and accurate copy of my own vaccination record will be uploaded in accordance with the instructions provided by the Landau Lawyers League.
I grant to the Landau Lawyers League, its representatives, affiliates, and/or agents the right to take photographs, record video or film the likeness of me and my spectators, with or without my knowledge. I authorize the Landau Lawyers League to copyright, use and publish the same in print and/or electronically. I agree the Landau Lawyers League may use such photographs and videos of me and my spectators with or without my name and for any lawful purpose, including but not limited to publicity, illustration, advertising and web content.
To the maximum extent permitted by law, under no circumstances shall the Released Parties be liable for any direct, indirect, incidental, special, consequential, or punitive damages from any security breach or any other security intrusion, electronic or otherwise. No data transmissions over the internet can be guaranteed to be 100% secure. Consequently, I understand the Released Parties cannot guarantee or warrant the security of any information transmitted to them so I do this at my own risk. The Released Parties do not guarantee the security of the portal or the services or the prevention from loss of, alteration of, or improper access to, my information or data.
I understand that I cannot play in the Landau Lawyers League until this Waiver form has been completed, including the necessary documents submitted. An e-mailed copy of this waiver is legal and binding. No representations, statements or inducements apart from this waiver have been made.
LANDAU LAWYERS LEAGUE LIABILITY WAIVER AND RELEASE - COVID AWARENESS AND RESPONSIBILITIES
I hereby forever voluntarily release, discharge, waive and relinquish any and all actions or causes of action against the Released Parties related to or arising from exposure to or from contracting COVID-19, any variants or mutations that cause COVID-19 and/or other illness or communicable disease of any type or severity or the causes thereof (all of the foregoing hereinafter referred to as “Communicable Diseases”) in and around any part of Los Angeles Unified School District or City of Los Angeles grounds or parking areas and/or the premises of any other property used and/or permitted by the Landau Lawyers League. By participating in any Landau Lawyers League game or event, I acknowledge and confirm, both now and in the future, that I understand and expressly assume the risk that I may be exposed to COVID-19 or other Communicable Diseases. I expressly understand that these risks include contracting COVID-19 or other Communicable Diseases and the related risks, physical and mental injuries, both foreseen and unforeseen, that may result from contracting COVID-19 or other Communicable Diseases, including death. I further acknowledge and understand that any interaction with the general public poses an elevated, inherent risk of being exposed to and contracting Communicable Diseases, including, but not limited to, COVID-19, that it cannot be guaranteed that I will not be exposed, and that as such, potential exposure to or contraction of COVID-19 or other Communicable Diseases are risks inherent in my decision to participate in Landau Lawyers League games or events that cannot be eliminated. I further acknowledge and understand that, if infected with COVID-19 or other Communicable Diseases, I may infect others I subsequently come in contact with, even if I am not experiencing or displaying any symptoms of illness, and that the risk of transmitting COVID-19 or other Communicable Diseases to others remains at all times. Accordingly, I further agree that I will not attend any Landau Lawyers League game or activity, if any of the following apply:
1) Currently or within the past fourteen (14) days, I have been diagnosed with COVID-19, or have or had a suspected diagnosis of COVID-19; OR
2) Currently or within the past fourteen (14) days, I have exhibited any symptom of COVID-19 or other Communicable Disease (which include but are not limited to fever, chills, cough, shortness of breath, difficulty breathing, fatigue, muscle aches, body aches, headache, new loss of taste or smell, sore throat, congestion or runny nose, nausea, vomiting, diarrhea); OR
3) Currently, I am in isolation or quarantine for any reason related to COVID-19 or other Communicable Diseases; OR
4) Currently, I am awaiting the result of a test for COVID-19 or other Communicable Disease.
DUTY TO SELF-MONITOR: I agree to self-monitor for signs and symptoms of COVID-19 including checking my temperature at home prior to attending any LLL game or event and will not attend if my temperature is at or over 100.4°F. While participating in games or events held by Landau Lawyers League, consistent with CDC guidelines, participants are encouraged to wear masks, social distance and to wash and sanitize hands regularly to reduce the risks of exposure to COVID-19. If, however, I observe any unusual or significant hazard during my presence or participation, I will remove myself and bring such observance to the attention of the nearest official immediately. I will immediately contact Landau Lawyers League at LandauLawyersLeague@gmail.com if, within 14 days after participating in any LLL Basketball or Softball Game or event, (1) I experience symptoms of COVID-19 or (2) I learn or suspect that, within 14 days prior to my participation in said game or event, I was or may have been exposed to an individual who has tested positive for COVID-19 or other Communicable Diseases,. I acknowledge that the Landau Lawyers League must contact the Los Angeles County Department of Public Health (LACDPH) to provide details of the confirmed positive test, including my name and contact information. I consent to the Landau Lawyers League providing such information to LACDPH or any other administration as required by law. I agree to willingly cooperate with any contact tracing that is deemed necessary by the Landau Lawyers League, LACDPH, and/or any other government authority.
I acknowledge the Governor, State Department of Health, LACDPH, Los Angeles Unified School District, City of Los Angeles or any other administration in the county of Los Angeles or otherwise with authority over the Landau Lawyers League may cancel a game or the season at any time. I also acknowledge that by participating in the Landau Lawyers League that I must comply with any mandates issued by any entity with the authority over athletics and agree to comply with any such directives even if issued after signature of this waiver. I further acknowledge and understand that my failure to do so may result in me being refused participation in or attendance at any game or event, and/or the entire sport season.
I am voluntarily participating in Basketball or Softball games, and I am aware of risks of participating in athletics during the COVID-19 pandemic. I agree that this section is intended to be a binding and full waiver of claims and release of liability related to exposure to or infection by COVID-19 or other Communicable Diseases. I agree to protect, defend, indemnify, save, and hold harmless the Released Parties from any and all liability, loss, damage, cost or expense arising from my negligence or reckless or willful misconduct in connection with my use or occupancy of Los Angeles Unified School District or City of Los Angeles premises and facilities and/or any other premises and facilities utilized by me in connection with the above activities.

UNDER PENALTY OF PERJURY, I acknowledge that I have read the foregoing paragraphs, have been fully and completely advised concerning same and am fully aware of all legal consequences of signing this document.
