Age Policy:
We do not allow spa, color or texture services for children under 16. Age 16-18 are permitted with a signed parental consent form on file, which must be completed prior to the appointment.
Guidelines:
● Minors (all clients under the age of 18) will only receive spa, color or texture care services with written parental or legal guardian consent.
● In collaboration with the consenting adult and child, the service provider will assist in establishing goals, and if it is a color/texture service, an estimated cost for the session.
● For ages 16-18, the presence of a parent or legal guardian in the treatment room or on the cutting floor is encouraged.
● Appropriate draping will be used at all times during the massage, only areas being massaged will be uncovered.
By signing yearly below, you agree to the following:
1) I am the parent/legal guardian of the minor receiving spa or salon services. I have read the above information and give permission for my child, age 16-18, to receive these treatments from Habitude service providers.
2) I understand that massage and or spa services are not a substitute for traditional medical treatment.
3) I understand that Spa Service Providers do not diagnose illness or injuries.
4) I understand the risks associated with massage and spa services may include, but are not limited to: superficial bruising, short-term muscle soreness, exacerbation of undiscovered injury, skin breakouts or irritation, or allergic reactions.
5) I understand the risks associated with hair texture and color services for minors and that skin breakouts, irritation or allergic reactions could occur.
6)I understand that prior color or chemicals in the hair can affect the service and it may take several visits to get the desired results.
I therefore release Habitude Inc. and their staff from all liability concerning these risks that may occur during their visit and understand that my child or the spa provider may terminate the session at any time.
