THE MICHAEL ELLIS SCHOOL FOR DOG TRAINERS AGREEMENT AND RELEASE FROM LIABILITY
1. Voluntary Participation. I acknowledge that I have voluntarily entered the property of The Michael Ellis School for Dog Trainers. I further acknowledge that activities on this property involve physical activity and being in close proximity to dogs that are being trained and, often, encouraged to attack and bite people. I further acknowledge that I will be in close proximity to numerous dogs, some of which are owned by other people present; The Michael Ellis School for Dog Trainers has no knowledge of those dogs’ character and temperament.
2. Assumption of Risk. I AM AWARE THAT TRAINING DOGS AND BEING IN THE PROXIMITY OF DOGS ARE HAZARDOUS ACTIVITIES. I AM FURTHER AWARE THAT BY PARTICIPATING IN THESE ACTIVITIES I RISK RECEIVING SEVERE INJURIES, INCLUDING BUT NOT LIMITED TO INJURIES CAUSED BY FALLING, DOG ATTACKS, AND/OR DOG BITES. I AM VOLUNTARILY PARTICIPATING IN THESE HAZARDOUS ACTIVITIES WITH KNOWLEDGE OF THE DANGER INVOLVED, HEREBY AGREE TO ACCEPT ANY AND ALL RISKS OF INJURY OR DEATH, AND VERIFY THIS STATEMENT BY PLACING MY INITIALS HERE:

3. Release. As consideration for being permitted by the Michael Ellis School for Dog Trainers to participate in these activities and use related facilities, I hereby agree that I, my assignees, heirs, distributees, guardians, and legal representatives will not make a claim against, sue, or attach the property of the Michael Ellis School for Dog Trainers, its owners, employees, agents, contractors or volunteers, on account of injury or damage resulting from the negligence or other acts, howsoever caused, by any owner, employee, agent, contractor or volunteer of the Michael Ellis School for Dog Trainers. I hereby release the Michael Ellis School for Dog Trainers, its owners, employees, agents, contractors and volunteers from all actions, claims, or demands that I, my assignees, heirs, distributees, guardians, and legal representatives now have or may hereafter have for injury or damage resulting from my presence on this property. 4. Current Covid-19 Guidelines. I agree to adhere to all current Sonoma County Covid-19 Safety Guidelines as implemented by The Michael Ellis School for Dog Trainers.

6. Campus Guidelines. I agree to follow the campus guidelines and rules provided. Failure to do so will result in forfeiture of participation without refund.

7. Photo Release: I also understand photos and video of me and my likeness can be used by Micheal Ellis School and Ford K9 Media, and can be used on social media and advertising. Initial Here
8. Knowing and Voluntary Execution. I HAVE CAREFULLY READ THIS RELEASE AND FULLY UNDERSTAND ITS CONTENTS. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT BETWEEN ME AND THE MICHAEL ELLIS SCHOOL FOR DOG TRAINERS, AND I SIGN IT OF MY OWN FREE WILL. I FURTHER AGREE THAT THIS AGREEMENT AND RELEASE SHALL BE CONTROLLED BY AND INTERPRETED UNDER THE LAWS OF THE STATE OF CALIFORNIA.
