RISKS

• CAUTION!   The Russian Sauna (Parilka) can reach temperatures of 220 or more.  The walls, floor, brick and metal surfaces are extremely hot; use extreme caution. 
  
• KEEP HYDRATED; sweat bathing & hydrotherapy are dehydrating; drink water; water available inside.  
  
• SLIPPING IS A HAZARD The spa is a wet area wear sandals at all times and use caution on stairs.

HEALTH CONCERNS 

• High contrast hydrotherapy is a vigorous physical activity; if you have any health conditions (diabetes, heart conditions, anemia, high or low blood pressure) or are pregnant please first consult with your physician before using the facility. 
  
• If you have any active infections or any communicable diseases, please refrain from using the facility or receiving services. 
• If you feel dizzy, nauseated, or sustain an injury immediately stop use of the facility and advise a Banya 5 Staff Member.

RULES & CONDUCT

• Shower before entering the sauna, pools, or steam room. Oils, perfumes, and lotions need to be cleansed from the skin before entering. Please wait to apply scents (deodorant, perfumes, oils, lotions, etc…) until you leave the facility.
• No PDA - Sexual activity, Lewd conduct and open signs of affection are strictly prohibited within the facility. You will be asked to leave. 
  
• No thong style swim suits allowed.  Full bottom and top coverage swimwear is required. Loaner bathing wear is available at the front desk. 
• Minor children must be supervised at all times and are the responsibility of parent or guardian.
• We are not responsible for any lost or stolen items. A lock box is provided at front desk. Change room lockers are keyed and if lost a $10 charge will be applied at checkout 
• If you are menstruating, we ask that you use a tampon before entering the pools.
• If you are under the influence of alcohol or narcotics, or exhibit any inappropriate behavior (spitting, profanity, derogatory statements, etc.) you will be asked to leave. 
• No food or alcohol is allowed into the facility. Water, tea, or juice must be in a plastic or paper container with a lid.  No glass containers are allowed.  
  
• I agree to abide by all present and future Banya 5 policies, rules, and regulations. 
  
• If I choose to receive a spa service, I certify that I have no conditions that are contraindicated for or would be aggravated by massage, aesthetics and/or salt/honey scrubs.  All body workers at Banya 5 are licensed with the state of Washington. 
• Banya 5 requires a 24-hour notice for all appointment cancellations.  Failure to provide this notice will result in an automatic charge up to the full amount of the services cancelled.
  
• Electronic devices are not allowed in the spa area. NO cell phones or computers of any kind.

RELEASE OF LIABILITY

I waive my right to sue and agree to defend, indemnify, and hold Banya 5, its’ owners, officers, employees, and agents harmless from lawsuits brought by myself or my heirs, from and against any and all liability, loss, expense (including reasonable attorneys' fees), or claims for injury or damages now or in the future that may arise out of my use of Banya 5’s facilities, including but not limited to the parking lot, locker room, wet area, steam, sauna, treatment rooms and common areas; or out of my participation in Banya 5 programs or services.

*If client is under the age of 18, the parent or legal guardian must be present and sign the consent form.
