I understand and agree that neither West Clayton Church of God (hereinafter referred to as WCCG), designated group leader(s), or the organization(s) sponsoring trips/activities may be held liable in any way for any occurrence in connection with said activities which may result in injury, death or other damages to me or my family, heirs, or assigns and in consideration for being allowed to participate in this activity.
I hereby personally assume all risks in connection with activities conducted by WCCG, including but not limited to activity content, location, supervision, equipment defects or malfunctions, damage to my person or property, temporary or permanent, medical expenses, further loss of wage or income, or any other economic loss, for any harm, injury or damage which may befall me while participating in said activities, including all risks connected therewith, whether foreseen or unforeseen; and further to save and hold harmless said organization and persons from any claim by me, or my family, estate, heirs, or assigns arising out of my or the listed participants (for which I am the legal parent or guardian) participation in said activities.
I further state that I am of lawful age, or if under lawful age, have the permission of my legal parent or guardian as acknowledged by the signature below, and legally competent to sign this affirmation and release.
By signing this affirmation and release, I further agree that I and the listed participants (for which I am the legal parent or guardian) are in good health, and know of no reason that we should not participate in activities, and that we all have seen a physician recently who can certify these facts.
I understand that the terms herein are contractual and not a mere recital; and that I have signed this document of my own free act.
This waiver shall be in effect for all activities and events held by WCCG until January 1st of the year following the date signed.
