PEMF Wellness Waiver
Wellness service (not veterinary care). I understand MagnaWave/PEMF is a wellness service for animals and is not veterinary diagnosis or medical treatment. No results are guaranteed. Animals may experience temporary responses such as relaxation/sleepiness, mild soreness, stiffness, increased drinking/urination, or changes in behavior/energy for 24–72 hours.
Assumption of risk (animals & property). I understand animals can react unpredictably. Risks include sudden movement, pulling back, striking, kicking, biting, rearing, and other behavior that may cause injury or property damage. I assume all risks related to my animal’s participation, handling, and the session environment (including off-site locations).
Health/behavior disclosure. I will inform the Provider before the session of any relevant conditions or concerns, including (but not limited to) pregnancy, fever/infection, recent surgery/acute injury, active bleeding/blood clots, cancer/tumors or treatment, implanted devices, seizures, or known dangerous behavior (kicking/biting/striking/pulling back)
Release of liability. I release and hold harmless Tower Hill Stables and its owners, employees, and contractors from any claims, damages, or injuries arising from the session or my animal’s participation, including negligence (except gross negligence or willful misconduct).
Authority & responsibility. I confirm I am the owner or authorized agent for the animal listed above and I am responsible for safe handling/control of my animal as requested. I agree to follow all safety instructions during the session.
Right to stop/refuse session for safety. I understand the Provider may stop or refuse a session at any time if my animal becomes unsafe to handle.
Illinois Equine Activity Liability Act. WARNING Under the Equine Activity Liability Act, each participant who engages in an equine activity expressly assumes the risks of engaging in and legal responsibility for injury, loss, or damage to person or property resulting from the risk of equine activities.
Photo/Video Permission. Yes, Tower Hill Stables may use photos/videos of my animal from the session for marketing.
