IMPORTANT INFORMATION: PLEASE REVIEW
Thank you for registering for the UAFS Intramural Sports Program! Here are a couple important things to know before your season begins:
1. Your team's schedule, along with the playing rules for the season, will be emailed to your UAFS email no later than the Monday before your first game, probably sooner. It is recommended that you download the Outlook app on your phone and login using your UAFS email and credentials. The schedule is a live document, and you will be able to see it in real time as it is updated to accommodate for any reschedules that take place.
2. Reschedules. Any team needing to reschedule a game must do so no later than 24 hours BEFORE the originally scheduled time. Captains (contact info found on the schedule) must consult with each other and agree upon a new date/time before contacting the Sports and Adventure Coordinator or an intramural referee. If no new time is agreed upon, the game will be forfeited by the unavailable team. PLEASE RESPECT THE TIME OF YOUR OPPONENTS AND THE INTRAMURAL SPORTS STAFF. Failure to appear for a game on more than one occasion without notifying the Intramural Sports Staff will result in the team forfeiting the season.
3. CAPTAINS: YOU are responsible for your team's communication, rescheduling, and ensuring they adequately understand all rules upon the start of competition. If you do not wish to have these responsibilities, please give the title of Captain to a teammate and notify the Intramural Sports Staff.

Intramural Waiver and release Acknowledgment
The undersigned student (hereinafter STUDENT) hereby acknowledges that he/she is voluntarily participating in a Intramural and/or club sport activity (hereinafter ACTIVITIES) at University of Arkansas Fort Smith (hereinafter UA FORT SMITH), is in good physical condition, and is sufficiently trained to participate in the activities.
In consideration of UA FORT SMITH making the activity available and/or making any equipment or facilities available, STUDENT, for himself or herself, his or her spouse, legal representatives, heirs, next of kin and assigns hereby forever releases, waives, and covenants not to sue UA FORT SMITH, its officers, Board of Trustee of the University of Arkansas, officers, agents, and employees from any and all liability to STUDENT, his or her spouse, legal representatives, heirs and assigns for any and all loss, claims, damages, lawsuits, demands, or causes of action whatsoever that STUDENT may have hereafter accrue, as the direct or indirect result of STUDENT’S participation in activities.
STUDENT acknowledges that these activities are each potentially hazardous. STUDENT understands that while participating in these activities, he or she will be exposed to above-normal risks of injury and that although UA FORT SMITH has taken precautions to provide safety equipment for each activity, it is impossible for UA FORT SMITH to guarantee absolute safety. STUDENT understands that he or she bears the responsibility for safety while participating in any intramural or club sport activity and voluntarily assumes full responsibility for the risk of bodily injury, death, medical expenses, loss of income, or property damage while participating in the activities, whether it is due to the active or passive negligence or otherwise of UA FORT SMITH STUDENT acknowledges that he/she has a personal responsibility to follow established rules of safety, obey all laws, fully utilize the safety equipment, provided for these activities, and to follow the instructions and commands of UA FORT SMITH employees or officials during participation in any of these activities. STUDENT acknowledges that he or she has been fully advised of the potential hazards that might occur include cuts, sprains, bruises, fractures internal injury, disfigurement (permanent or otherwise), blindness, paralysis, suffocation, broken bones, and other serious or even life-threatening injuries.
With all of these facts known, STUDENT voluntarily agrees to release UA FORT SMITH, its officers, Board of Trustee of the University of Arkansas, officers, agents, and employees from any and all liability to student, his or her spouse, legal representatives, heirs and assigns for any and all loss, claims, damages, lawsuits, demands, or cause of action whatsoever that participant may have or that my hereafter accrue, as the direct or indirect result of STUDENT’S participation in recreational and/or club sport activities.
It is expressly understood by STUDENT that he/she is solely responsible for all costs arising out of any bodily or property damage sustained through his/her participation in the activities. STUDENT expressly agrees that this release is intended to have broad and inclusive as permitted by the laws of the State of Arkansas, and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect. Further, nothing in this document shall be deemed to waive the sovereign immunity of the State of Arkansas, UA FORT SMITH, Board of Trustee of the University of Arkansas, officers, employees, staff, or agents.
STUDENT also gives permission to UA FORT SMITH for the free use of STUDENT’S name and/or pictures in broadcasts, telecasts, publication, and newspapers.

THIS DOCUMENT RELEASES UNIVERSITY OF ARKANSAS FORT SMITH AND BOARD OF TRUSTEES OF THE UNIVERSITY OF ARKANSAS FROM ANY LIBAILITY RESULTING FROM MY PARTICIPATION IN ANY INTRAMURAL/RECREATIONAL SPORT ACTIVITES.
