YOUTH Athlete Waiver Release

 In consideration of being allowed to participate (in any way) in Seattle Adaptive Sports (SAS) programs, related events and activities, I and/or the minor participant, for myself, and on behalf of my heirs, assigns, personal representatives and next of kin, the undersigned:

1. Agree that prior to participating, I will inspect (or if a parent and/or legal guardian I will instruct the minor participant to inspect) the facilities and equipment to be used, and if I believe, to the best of my ability, that anything is unsafe, I and/or the minor participant will immediately advise the coach or team manager in charge of such condition(s) and refuse to participate.

2. Acknowledge and fully understand that I and/or the minor participant will be engaging in activities that involve risk of serious injury, including permanent disability and death, and severe social and economic losses which might result from my own actions, inactions, or negligence from others, the rules of play, or the conditions of the premises or any equipment used. Further, that there may be other risks not known to me or not reasonable foreseeable at the time.

3. Assume all the foregoing risks and accept personal responsibility for the damages following such injury, permanent disability, and/or death.

4. Release, waiver, discharge and covenant not to sue Seattle Adaptive Sports, Seattle Parks and Recreation, Sno-King Hockey, Tennis Center at Sand Point, Seattle School District, US Paralympic, NWBA, USA Hockey, USPSA, USTA, ASUSA, DSUSA, their representative administrators, directors, agents, coaches, volunteers and other staff of the organization, other participants, sponsoring agencies, sponsors, advertisers, their heirs, and if applicable, owners and leasers of premises used to conduct the event, all of which are hereinafter referred to as “Releasees”, from demands, losses, or damages on account of injury, including death or damage to property, caused or alleged to be caused in whole or in part by the negligence of the Releasee or otherwise.

5. Permission is hereby granted to the athletic trainer(s) of Seattle Adaptive Sports should any injury occur to provide athletic training services in the form of emergency assessments and/or management of any acute sports injury suffered. An emergency referral to an appropriate physician if, in the opinion of the athletic trainer, treatment or further evaluation is necessary is also consented. Permission is hereby granted to the athletic trainer(s) of Seattle Adaptive Sports to provide athletic training services in the form of care, treatment, evaluation, management, and rehabilitation of any acute or chronic sports injury suffered by the athlete

FOR PARTICIPANTS UNDER THE AGE OF 18 This is to certify that I, as parent/guardian with legal responsibility for this participant, do consent and agree to his/her release as provided above of the Releasees, and, for myself, my heirs, assigns, and next of kin, I release and agree to indemnify and hold harmless the Releasees from any and all liabilities incident to my minor child’s involvement or participation in these programs as provided above, even if arising from their negligence.

Athlete Code of Conduct

 To be read and signed by all parents of players who are members of Seattle Adaptive Sports.

1. I will not force my children to participate in sports, but support their desires to play their chosen sport. Children are involved in organized sports for their enjoyment, not mine. 

2. I will make it fun for all children participating. 

3. I will applaud a good effort in victory and in defeat and enforce the positive points of the game. 

4. I will work toward removing the physical and verbal abuse in youth sports.

5.  I will enjoy the game, learn all I can about the game, and volunteer! 

6. I will encourage my child to play by the rules. Remembering, that my child learns best by example, I will applaud the good plays of both teams. 

7. I recognize the importance of volunteer coaches. They are very important to the development of my child and the sport. I will allow them to coach my child. I will communicate with them and support them. 

8. When talking to a child about his or her participation and performance, I will discuss the positive contributions made not areas for performance improvements. 

9. I will know and study the rules of the game, and support the officials on and off the court/ice. This approach will help in the development and support of the game. Any criticism of the officials only hurts the game. 

10. I will respect the facilities/properties where teams have games/practices, their employees and any rules. 

11. I will not discuss games or circumstances of a game with our player's coach within 24 hours of completion, on the grounds of being dissatisfied with said coach. 

12. I will not embarrass my child by yelling at players, coaches or officials. (Hockey: I will not pound on the glass.) By showing a positive attitude toward the game and all of its participants, my child will benefit.

By adhering to the terms of my Parental Codes of Conduct, I realize that I will create a fun and positive experience for all those involved in the opportunity for adaptive sports.

I further understand and agree that non-adherence to these terms may result in disciplinary action up to and including expulsion from Seattle Adaptive Sports activities. Members ejected from a game will receive at minimum, a 2 week suspension.

Permission Document

I hereby acknowledge and give permission for  the minor child whose welfare I am legally responsible for, to participate and travel with my Seattle Adaptive Sports team, their staff, coaches, and volunteers for the purpose of competition during the 2023 – 2024 season. I further agree to hold harmless Seattle Adaptive Sports, NWBA, USA Hockey, USPSA, USTA, USTFA, US Olympic/Paralympic Organization, Adaptive Sports USA, volunteers, directors, facilities, staff and the city from any and all demands, claims, expenses or liability of any nature caused by my acts or omissions while participating at this event. I understand and agree that Seattle Adaptive Sports, or other supporting parties are not responsible for, but not limited to any injury or property damage arising out of competing in these games, even if caused by their ordinary negligence or otherwise. Permission is also given for Seattle Adaptive Sports, participants and/or volunteers to seek or render medical aid for my child in case of a medical emergency

 Media/Photo Release

I hereby authorize and give my full consent to Seattle Adaptive Sports to copyright and/or publish any and all photographs, videotapes and/or film in which I appear while attending SAS practices, events and/or activities. I further agree that SAS may transfer, use or cause to be used, these photographs, videotapes, or films for any exhibitions, public displays, publications, commercials, art and advertising purposes, and television programs without limitations or reservations.
