Seattle Adaptive Sports ADULT ATHLETE Waiver/Release of Liability, Code of Conduct & Media Release Agreements

ADULT Athlete Waiver Release

 In consideration of being allowed to participate (in any way) in Seattle Adaptive Sports (SAS) programs, related events and activities, I and/or the minor participant, for myself, and on behalf of my heirs, assigns, personal representatives and next of kin, the undersigned:

1. Agree that prior to participating, I will inspect (or if a parent and/or legal guardian I will instruct the minor participant to inspect) the facilities and equipment to be used, and if I believe, to the best of my ability, that anything is unsafe, I and/or the minor participant will immediately advise the coach or team manager in charge of such condition(s) and refuse to participate.

2. Acknowledge and fully understand that I and/or the minor participant will be engaging in activities that involve risk of serious injury, including permanent disability and death, and severe social and economic losses which might result from my own actions, inactions, or negligence from others, the rules of play, or the conditions of the premises or any equipment used. Further, that there may be other risks not known to me or not reasonable foreseeable at the time.

3. Assume all the foregoing risks and accept personal responsibility for the damages following such injury, permanent disability, and/or death.

4. Release, waiver, discharge and covenant not to sue Seattle Adaptive Sports, Seattle Parks and Recreation, Sno-King Hockey, Tennis Center at Sand Point, Seattle School District, US Paralympic, NWBA, USA Hockey, USPSA, USTA, ASUSA, DSUSA, their representative administrators, directors, agents, coaches, volunteers and other staff of the organization, other participants, sponsoring agencies, sponsors, advertisers, their heirs, and if applicable, owners and leasers of premises used to conduct the event, all of which are hereinafter referred to as “Releasees”, from demands, losses, or damages on account of injury, including death or damage to property, caused or alleged to be caused in whole or in part by the negligence of the Releasee or otherwise.

5. Permission is hereby granted to the athletic trainer(s) of Seattle Adaptive Sports should any injury occur to provide athletic training services in the form of emergency assessments and/or management of any acute sports injury suffered. An emergency referral to an appropriate physician if, in the opinion of the athletic trainer, treatment or further evaluation is necessary is also consented. Permission is hereby granted to the athletic trainer(s) of Seattle Adaptive Sports to provide athletic training services in the form of care, treatment, evaluation, management, and rehabilitation of any acute or chronic sports injury suffered by the athlete

Athlete Code of Conduct

 The purpose of this Code is to promote the best possible team and individual impression at all times, and to acknowledge each athlete’s individual responsibility to the team. Each athlete must read and sign this Code of Conduct, agreeing to the articles within, in order to participate as a member of any SAS team.

1. The possession of alcohol, tobacco products, or controlled/uncontrolled substances is strictly prohibited.

2. Curfews established by the coaches will be followed.

3. There will be no male athletes in female athletes’ rooms and no female athletes in male athletes’ rooms unless supervised by a coach.

4. Team members and staff will attend all team functions including meetings, warm-ups, practices, competitions, team meals, etc., unless excused or instructed by a coach.

5. Players are expected to be on time for all events, practices, games or team functions.

6. Players are expected to participate to the best of their ability at all practices, games, and team functions unless prior approval from the coach is received. Players are expected to call or make every effort necessary to get in touch with the coach if he/she cannot attend.

7. Team members and staff will refrain from any illegal or inappropriate behavior that will distract from competitions or tarnish the positive image of SAS.

8. Team members will display proper attitude, respect, and sportsmanship towards coaches, officials, fellow competitors, event staff, and the public.

9. Team member must maintain all equipment and uniforms provided by SAS and understand that all equipment and uniforms are property of SAS and must be returned when the athlete is no longer with the program.

10. Proper attire must be worn at all times and each individual must dress responsibly and maintain personal hygiene.

11. Any damage or loss to public or private property will be paid for by those persons responsible for the damage or loss that occurs. Parents are responsible for their children’s financial obligations.

12. Junior team members will be enrolled in school and verify enrollment upon request. Each athlete will attend school at least 90% during each quarter/semester and hold a C grade of higher in each class to participate. This confirmation will be taken from school records. School records will be checked regularly. Accuracy is your responsibility.

13. Traveling teams will follow the team expectation of SAS and team coaches. This is individual to each team and will be reviewed with your team member by the team coaches.

14. All coaches’ decisions are final. Team members have the right to appeal decisions to the SAS board for review.

15. Any additional guidelines for the team will be established as needed.

16. Sled Players: Sno-King Amateur Hockey Associate has additional guidelines and policies.

IMPLEMENTATION: I have received the Athlete’s Code of Conduct of Seattle Adaptive. By signing this document, it is acknowledged that I have read it, understand it, and am willing to abide by the codes listed within. By signing this document it is further acknowledged that I am a representative of SAS and my sports team and I agree to conduct myself with honesty, integrity, and exemplary behavior. Furthermore, I understand that violation of any of the codes can/will result in disciplinary action which may include, but is not limited to, suspension or removal from the team and immediate return home (at parents’/athlete’s expense).

Permission Document

I hereby acknowledge that I am participating and traveling with my Seattle Adaptive Sports team, their staff, coaches, and volunteers for the purpose of competition during the 2023 – 2024 season. I further agree to hold harmless Seattle Adaptive Sports, NWBA, USA Hockey, USPSA, USTA, USTFA, US Olympic/Paralympic Organization, Adaptive Sports USA, volunteers, directors, facilities, staff and the city from any and all demands, claims, expenses or liability of any nature caused by my acts or omissions while participating at this event. I understand and agree that Seattle Adaptive Sports, or other supporting parties are not responsible for, but not limited to any injury or property damage arising out of competing in these games, even if caused by their ordinary negligence or otherwise. Permission is also given for Seattle Adaptive Sports, participants and/or volunteers to seek or render medical aid for my child in case of a medical emergency

 Media/Photo Release

I hereby authorize and give my full consent to Seattle Adaptive Sports to copyright and/or publish any and all photographs, videotapes and/or film in which I appear while attending SAS practices, events and/or activities. I further agree that SAS may transfer, use or cause to be used, these photographs, videotapes, or films for any exhibitions, public displays, publications, commercials, art and advertising purposes, and television programs without limitations or reservations.
