Consent for Treatment, Outbound Trips, and Photographs
The Parent(s) and/or Guardian(s) of the minor participant hereby give consent and permission to the following NLArts Community Programming provisions:
Emergency Medical Care:
The participating minor has permission to receive emergency medical treatment by qualified medical personnel, including a physician or surgeon, in the event of sudden illness or injury during participation in NLArts activities. It is understood that NLArts does not provide medical insurance for such treatment, and any associated costs will be the responsibility of the parent/guardian.
Travel & Transportation
While accompanied by NLArts staff, the participating minor has permission to:
walk to and from nearby neighborhood locations;
use public transportation to and from locations within Philadelphia, as deemed appropriate;
use private transportation arranged by NLArts for field trips when other travel methods are not applicable.
Media Release:
The participating minor and any artworks created during NLArts activities may be photographed, video recorded, and/or audio recorded. NLArts and its approved guest artists, collaborators, or partner organizations have the right to document and use such media for educational and promotional purposes related to the program. NLArts reserves the right to publish, copyright, and share this media across platforms including (but not limited to) its website, social media, print materials, and partner communications. No compensation will be provided for the use of this media.
Self Sign-Out Permission
Participating minors may be permitted to sign themselves out of camp and walk home unaccompanied. Participants must sign out at the official end time of programming and follow any instructions provided by the parent/guardian.
