WAIVER & RELEASE
The undersigned has agreed to participate in one or more equine-related activities at MAPLE CREST FARM (MCF). I acknowledge that there are certain inherent risks regarding equine activities, including but not limited to:
- The propensity of an equine to behave in ways that may result in injury, death, or loss to persons on or around the equine;
- The unpredictability of an equine’s reaction to sounds, sudden movement, unfamiliar objects, persons or other animals;
- Hazards, including, but not limited to, surface or subsurface conditions;
- A collision with another equine, another animal, a person, or an object;
- The potential of an equine activity participant to act in a negligent manner that may contribute to injury, death, or loss to the person of the participant or to other persons, including, but not limited to, failing to maintain control over an equine or failing to act within the ability of the participant.
I hereby assume all risk of illness, injury or death to which I may be exposed to by participating in an activity or event at MCF.
I hereby discharge, waive, release from liability and hold harmless MCF, its successors and assigns, and any of its owners, employees and agents, from any damages as a result of illness, injury or death to myself or any of my property as a result of my participation in an activity or event at MCF. This waiver and release from liability, and all terms contained herein, shall also be binding on my dependents, heirs, executors, administrators and personal representatives.
By signing this Agreement, I acknowledge that I have read and fully understand this Waiver and Release. Further, I understand that by signing this Agreement I am giving up certain legal rights including the right to recover damages in case of injury, death or property damage arising from my participation in an equine-related activity or event at MCF.
I am further aware of the rules and regulations in effect at MCF and agree to abide by them, including any rules and regulations pertaining to the wearing of an equestrian helmet.

CONSENT TO MEDICAL TREATMENT
I hereby authorize MCF, any of its agents or employees, to contact any emergency medical services or a licensed physician, dentist or other qualified medical personnel acting under their supervision, if it becomes necessary to do so as a result of an illness, accident or injury to myself while I am participating in an activity or event at MCF.
I hereby authorize MCF, any of its agents or employees, to transport the undersigned to any urgent care center or emergency room if it becomes necessary to do so as a result of an illness, accident or injury sustained while participating in an activity or event at MCF.
I further authorize any first aid or emergency medical care prescribed by a duly licensed physician, dentist, or other qualified medical personnel acting under their supervision, that may become necessary as a result of an illness, accident or injury sustained while participating in an activity or event at MCF.
I agree to assume all financial costs, as a result of any illness, injury or death as a result of participation in an activity or event at MCF, and will further indemnify MCF, its successors and assigns, agents and employees for same.

I agree to provide MCF with updated health insurance information for myself. I acknowledge that the purpose of this is, that in
the event of an emergency, is to be able to provide a physician, dentist
or other medical personnel with any needed information to more
efficiently render care.

PHOTOGRAPHY & VIDEO
I agree to refrain from taking any photography or video without the consent of MCF. Further, I acknowledge that MCF may obtain photographs and/or video of myself, and further that said photographs and/or video may be used in promotional materials.

SAFETY HELMET AGREEMENT
Each
and every individual owner, leaser, student, and handler, must wear an approved helmet at Maple Crest Farm.
Riders are encouraged to help each other to police this effort while
catching, leading, grooming, riding, lunging, bathing, driving,
exercising, showing, and training the horses.
Our goal is to reduce the chances of a head injury due to the size and wight of the animals on this property. Horses are a herd animal, they are also prey animals, nature can sometimes takeover our domestication of equine.
If riders cannot follow this rule, it is within MCF's rights to excuse the rider from the property.
Thank you for your understanding and attention to this simple facility regulation.
