Mad House Boxing Wavier Form

PLEASE READ THE FOLLOWING CAREFULLY YOUR SIGNATURE ACKNOWLEDGES YOUR AGREEMENT TO ALL SECTIONS OF THIS WAIVER FORM EXPRESSING ASSUMPTION OF RISKS.

I the undersigned is aware that there are significant risks involved in all aspects of physical training and competitive sports training. These risks include but are not limited to head injury sports, and if the member sustains an injury such as a Concussion, Brain Bleed, Headache, Bloody Nose, Broken Nose, Broken Limbs, Broken Neck, Might become Paralyzed, Dislocated Limbs such as Shoulders, Knees Ankles, Elbows Fingers, Hips, Toes, or Jaw. The participant is-not limited to, Sprains, Strains, Broken Bones, Lacerations, Elevated Blood Pressure, Heartbeat Disorders, Fainting, Shortness of Breath, Chest Pain, Strokes, Heart Attack, or Accidental Death or death due to improper use or failure of equipment. The participant is willingly assuming full responsibility for the risk that they exposing them self to and accept. FULL RESPONSIBILITY AND IF THE PARTICIPANT IS UNDER THE AGE OF 18 THE PARENT OR GUARDIAN TAKE FULL RESPONSIBILITY FOR ANY INJURY OR DEATH MAD HOUSE BOXING AND SPORTS PERFORMANCE GYM LLC, I the undersigned acknowledge that I have no physical impairment or illnesses such as respiratory illness flu-like symptom and COVID-19 that will endanger myself or other.     

RELEASE OF LIABILITY

In consideration of the above mentioned risk and hazards, I am willingly and voluntarily participating in the activities available at Mad House Boxing Sports and Performance Gym, I the undersigned hereby release Mad House Boxing and Sports Performance Gym LLC or their principal’s agents, officers, employees, and volunteers from any and all liability, claims, demands, actions or rights of action, which are related to, arise out of, or are in any way connected with my participation in this activity, including those allegedly attributed to the negligent acts or omissions of the above-mentioned parties. This agreement shall be binding upon me, my successors, representatives, heirs, executors, assigns, or transference. If any portion of this agreement is held invalid, I agree that the remainder of the agreement shall remain in full legal force and effect. If I am signing on behalf of a minor child, I also give full permission to call for the medical and/surgical care for the child and to transport the child to a medical facility deemed necessary for the well-being of the child.

USA BOXING PASSPORT

Any member who wishes to spar or compete in the contact boxing class will attain there USA PASSPORT IN THE FIRST 30 DAY OF SIGNING THIS WAIVER FORM! AND WILL RENEW THEIR PASSPORT ONCE A YEAR TO KEEP THE BOOK UP TO DATE. IF THE BOOK IS NOT RENEWED THEY CANNOT PARTICIPATE IN ANY CONTACT BOXING/KICKBOXING/JIU-JITSU/OR ANY CONTACT TRAINING AT MAD HOUSE BOXING AND SPORTS PERFORMANCE GYM LLC.

PARTY RENTAL

BY SINGING THIS ALL FANTASY PARTY KNOWING TO KNOW THAT THEY’RE AGREEING TO PARTICIPATE IN CONTACT ACTIVITY AND WON’T NEED A USA BOXING PASSPORT FOR THE SUMO SUIT MATCHES, LARGE BOXING GLOVES MATCH, ANY OF THE WRESTLING MATCH OR BUMPER BALL MATCH. BUT AGREE TO ALL THE RULES IN THIS WAIVER FORM

Use of picture/film/likeness I further agree to allow Mad House Boxing and Sports Performance Gyms LLC, its agents, officers, Principle, employees and volunteers the use of a picture (s), film, or likeness for advertising purposes. In the event I choose not to allow the use of the same for said purpose, I agree that I must inform Mad House Boxing and Sports Performance Gym in writing.

INDEMNIFICATION

I, the undersigned, recognize that there is risk involved in the types of activities offered by Mad House Boxing And Sports Performance LLC. Therefore, I accept full financial responsibility for an injury that I may cause to myself or to any other participant due to my negligence. Should the above-mentioned parties, or anyone acting on their behalf, be required to incur attorney’s fees and costs. I further agree and cost to enforce this agreement, I agree to reimburse them for such fees and cost. I further agree to indemnify and hold harmless Mad House Boxing and Sports Performance Gym LLC, their principals, agents, officers, employees, and volunteers from liability for the injury or death of any person (s) and damage to property that may result from my negligent or intentional act or omission while participating in activities offered by Mad House Boxing and Sports Performance Gym LLC.
