I acknowledge and give permission to MGL Training, LLC and its contractors to use my likeness or my child(ren’s)’s likeness in a photograph or video in any and all of its publications, including but not limited to all of MGL Training, LLC’s printed and digital publications and social media platforms. I understand and agree that any photograph or video will become the property of MGL Training, LLC and will not be returned.
I acknowledge that my participation or that of my child(ren) is voluntary and I will receive no financial compensation. I hereby irrevocably authorize MGL Training, LLC and its affiliates to edit, alter, copy, exhibit, publish, or redistribute photos and/or videos for purposes of publicizing MGL Training LLC’s programs or for any other related, lawful purpose.
In addition, I waive the right to inspect or approve the finished product, including in written or electronic copy, wherein my likeness appears. Additionally, I waive any right to any royalties or other compensation arising out of, or related to, the use of the photograph or video.
I hereby hold harmless and release and forever discharge MGL Training, LLC and its affiliates from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my child(ren) or estate have or may have by reason of this authorization.
I, the signee, affirm that I am of the age of 18 years or older, and that I am freely signing this agreement. I certify that I have read this agreement, that I fully understand its content and that this release cannot be modified orally. I am aware that this is a release of liability and a contract and that I am signing it of my own free will.
