Lil Bunny Team Waiver Form

ASSUMPTION OF RISK, RELEASE AND WAIVER OF LIABILITY, INDEMNITY AGREEMENT, AND MEDICAL CONSENT (“WAIVER”)

In consideration of being allowed to enter and/or participate in any activities at Lil Bunny Play locations, operated by Primaeyu Entertainment LLC, the undersigned acknowledges and agrees to the following:

1. Representation of Authority
   I attest that I am the parent or legal guardian of the participant(s) named below, or that I have obtained authorization from the parent or legal guardian to execute this agreement on their behalf. I agree to supervise and monitor the activities of the participant(s) to ensure all activities are age-appropriate and ability-appropriate.
2. Compliance with Rules
   I agree that the participant(s) and I shall comply with all stated and customary terms, posted safety signs, rules, and instructions provided by Lil Bunny Play staff as conditions for using the equipment and participating in any events, activities, or programs.
3. Acknowledgment of Risks
   I, on behalf of myself and the participant(s), understand and acknowledge that all equipment at Lil Bunny Play, including but not limited to slides, stairs, climbing structures, inflatables, trampolines, and other toys, can pose risks of serious injury, illness, or death. I accept all risks associated with such activities, including those resulting from the negligence of others. I further agree to indemnify, defend, and hold harmless Primaeyu Entertainment LLC, its employees, and agents from all claims, damages, or losses arising from our participation. I confirm that I have adequate insurance to cover any medical expenses incurred.
4. Age Restrictions
   I understand that certain activities have age-specific restrictions to ensure the safety of participants.
5. Release and Waiver
   In consideration of being permitted to participate, I, for myself and the participant(s), release, waive, and discharge Primaeyu Entertainment LLC dba Lil Bunny Play, its owners, agents, and employees from all claims, demands, and liabilities arising out of our participation, including those caused by negligence.
6. Assumption of Responsibility
   I understand the legal effect of this waiver and agree to assume all risks of injury. I fully and forever release and discharge Primaeyu Entertainment LLC dba Lil Bunny Play from any claims arising from the use of its facilities or equipment.
7. Supervision Responsibility
   While Lil Bunny Play staff observe and monitor activities, I acknowledge that it is my responsibility to supervise my children. Primaeyu Entertainment LLC dba Lil Bunny Play is not responsible for ensuring the safety of children beyond its stated operational rules.
8. Facility Rules
   I agree to adhere to facility rules, including the prohibition of outside food and the requirement to wear socks at all times. Exceptions for medical conditions must be addressed with Lil Bunny Play staff. Failure to comply with these rules may result in denial of entry.
9. Media Consent
   I authorize Primaeyu Entertainment LLC dba Lil Bunny Play to use images or videos of myself and the participant(s) for marketing, security, or promotional purposes.
10. Refund Policy
    I understand that no refunds will be provided if I or the participant(s) remain on the premises for 20 minutes or more, regardless of circumstances.
11. Voluntary Participation
    Participation is voluntary, and I am not compelled to participate despite the risks. I acknowledge that I have carefully read and understand this agreement.
12. Behavioral Policy
    Lil Bunny Play staff reserves the right to enforce rules for safety and enjoyment. Disruptive behavior may result in warnings, timeouts, or expulsion without a refund.
13. Medical Authorization
    If the participant is a minor, I authorize Primaeyu Entertainment LLC dba Lil Bunny Play to seek emergency medical treatment if deemed necessary. I release Primaeyu Entertainment LLC dba Lil Bunny Play from liability related to medical treatment, including negligence, and agree to indemnify Primaeyu Entertainment LLC dba Lil Bunny Play for associated costs.

IMPORTANT NOTICE

• All equipment is designed exclusively for children. Parents or guardians are not permitted to use the equipment except for the large playground structure.
• Refusal to sign this waiver will result in denial of participation.
• By signing the waiver, I certify that I have read, understand, and agree to the terms of this waiver.
