I, [FIRST NAME][LAST NAME], hereby acknowledge that my child, [PARTICIPANT NAMES] has been registered to participate in the youth football camp hosted by The Huddle (“the Camp”) on May 18, 2024.
In consideration for my child being permitted to participate in the Camp, I, on behalf of myself and my child, agree to the following terms and conditions:
- Assumption of Risk: I acknowledge and understand that participation in the Camp involves inherent risks, including but not limited to physical injury, illness, and property damage. I understand that my child may be exposed to risks associated with physical activities, including but not limited to collisions, falls, and other participants' actions.
- Release of Liability: I hereby release, waive, discharge, and covenant not to sue The Huddle, its officers, employees, volunteers, agents, and representatives from any and all claims, demands, damages, actions, or causes of action arising out of or relating to any loss, damage, or injury, including death, that may be sustained by my child or any property belonging to my child, whether caused by the negligence of the Released Parties or otherwise, while participating in the Camp.
- Indemnification: I agree to indemnify and hold harmless the Released Parties from any loss, liability, damage, or cost, including attorney fees, that may occur as a result of my child's participation in the Camp, whether caused by the negligence of the Released Parties or otherwise.
- Medical Treatment: In the event of any illness, injury, or medical emergency involving my child during the Camp, I authorize the Camp staff to obtain medical treatment for my child and agree to be financially responsible for any costs associated with such treatment.
- Photography and Publicity: I consent to the use of my child's name, likeness, and/or image in photographs, videos, or other promotional materials related to the Camp without compensation or further notice.
I HAVE CAREFULLY READ THIS WAIVER AND FULLY UNDERSTAND ITS CONTENTS. I AM AWARE THAT BY SIGNING THIS WAIVER, I AM WAIVING CERTAIN LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE. I SIGN THIS WAIVER VOLUNTARILY AND WITHOUT INDUCEMENT.
