THIS IS A RELEASE AND ASSUMPTION OF RISK

In consideration of the acceptance of this registration request and my participation in this activity, I undersigned, intending to be legally bound, hereby for myself, my heirs, my assigns and my representatives, assume all risk attendant to participation in the activity and release and forever discharge Impact Archery Inc. its agents, associates, representatives, successors and assigns and any and all other sponsors, their representatives, successors and assigns, for any and all claims for damages which I may have or may hereafter accrue to me for property damages or personal injuries suffered by me in this activity. I attest and verify that I have full knowledge of the risk involved in participating in the activity. I attest and verify that I am physically fit and have sufficiently trained for this activity. I acknowledge that I have sole responsibility for my personal possessions and archery equipment. If I am signing this on behalf of a minor child, I further authorize the performance of emergency medical treatment for any injury sustained by the minor child during this activity.

PARENT CONSENT

I, the minor’s parent and /or legal guardian, understand the nature of the above referenced activities and the minor’s experience and capabilities and believe the minor to be qualified to participate in such activity. I hereby release, discharge, covenant not to sue and AGREE TO INDEMNITY AND SAVE AND HOLD HARMLESS each of the RELEASES from all liability, claims, demands, losses, or damages on the minor’s account caused or alleged to have been caused in whole or in part by the negligence of the RELEASES or otherwise, including negligent rescue operations, and further agree that if, despite this release, I, the minor, or anyone on the minor’s behalf makes a claim against any of the above RELEASES, I WILL INDEMNITY, SAVE AND HOLD HARMLESS each of the RELEASES from any litigation expenses, attorney fees, loss liability, damage, or cost any RELEASES many incur as the result of any such claim.

Impact Archery INC requires all shooting participants to wear proper footwear while participating in any shooting activities. Proper footwear is considered sturdy, closed toe shoes. Should a participant chose to shoot without wearing the proper footwear, theparticipant assumes all liability and risks. Impact Archery INC and it's employees will not be responsible for any damages or injuries that may arise from participant not proper footwear. Some examples of improper footwear are but not limited to: sandals, flip-flops, shoes with an extended heel.

TALENT RELEASE FORM

The undersigned enters into this Agreement with Impact Archery INC, herein referred to as Producer. I have been informed and understand that Impact Archery INC is producing social media content that my contain my name, likeness, image, voice, appearance and/or performance is being recorded and made a part of that production ("Product").

 I grant Impact Archery INC and its designees the right to use my name, likeness, image, voice, appearance, and performance as embodied in the Product whether recorded on or transferred to videotape, film, slides, photographs, audio tapes, or other media, now known or later developed. This grant includes without limitation the right to edit, mix or duplicate and to use or re-use the Product in whole or part as Impact Archery INC may elect. Impact Archery INC or its designee shall have complete ownership of the Product in which I appear, including copyright interests, and I acknowledge that I have no interest or ownership in the Product or its copyright.

 I also grant Impact Archery INC and its designees the right to broadcast, exhibit, market, sell and otherwise distribute the Product, either in whole or in parts, and either alone or with other products, for commercial or non-commercial television or theater, closed-circuit exhibition, home video distribution, Internet or any other purpose that Impact Archery INC or its designees in their sole discretion may determine. This grant includes the right to use the Product for promoting or publicizing any of the uses.

 I confirm that I have the right to enter into this Agreement, that I am not restricted by any commitments to their parties, and that Impact Archery INC has no financial commitment or obligations to me as a result of this Agreement. I hereby give all clearances, copyright and otherwise, for use of my name likeness, image, voice, appearance and performance embodied in the Product. I expressly release and indemnify Impact Archery INC and its officers, employees, agents and designees from any and all claims known and unknown arising out of or in any way connected with the above granted uses and representations. The rights granted Impact Archery INC herein are perpetual and worldwide.

I hereby release and agree to hold Impact Archery INC harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the archery range/shop, or that may otherwise arise in any way in connection with any services received from Impact Archery INC. I understand that this release discharges Impact Archery INC from any liability or claim that I, my heirs, or any personal representatives may have against the business with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from Impact Archery INC. This liability waiver and release extends to the archery range/shop together with all owners, partners, and employees.
