WAIVER AND INDEMNIFICATION
TO: East Park Golf Gardens Limited
In consideration of participating in activities organized by East Park Golf Gardens Limited, including but not limited to bootcamp and yoga activities, I, hereby acknowledge the following:
I understand that participation in physical activities like bootcamp and yoga carries inherent risks, including but not limited to muscle strains, sprains, and other injuries.
I acknowledge that I am physically fit to participate in these activities and have no medical condition that would prevent me from safely engaging in physical exercise. I understand that it is my responsibility to consult with a physician prior to participating if I have any concerns about my health or fitness level.
In consideration of being permitted to participate in bootcamp and yoga activities, I hereby waive, release, and discharge East Park Golf Gardens Limited, its instructors, and any other individuals associated with the activities from any and all claims, liabilities, damages, or expenses that may arise as a result of my participation, including but not limited to those arising from negligence or carelessness on the part of the organizers or instructors.
I understand that photographs and/or videos may be taken during the bootcamp and yoga activities, and I grant permission for my likeness to be used for promotional purposes.
I have read this waiver and fully understand its terms. I voluntarily agree to its content and sign it of my own free will.
By agreeing to this waiver, you agree that you have read and accept the terms of the waiver and that the information you provided is accurate. You furthermore agree that your submission of this form, via the 'I Agree' button, shall constitute the execution of this document in exactly the same manner as if you had signed, by hand, a paper version of this agreement.
