Child Permission Form/Waiver of Liability
I, the parent of “my child”, give permission for my child to attend.
I understand that personal injury can and may occur to my child, and I hereby authorize below adult, to seek and consent to emergency medical attention for my child as needed; and I further agree to be liable for and to pay all costs incurred in connection with such medical attention.
I hereby release Dreamland Dripping Springs, Dreamland at Tank Town LLC, SLK Real Estate LLC and their owners, employees, agents and animals, from any and all liability, claims, demands, causes of action and possible causes of action whatsoever including but not limited to negligence and gross negligence claims arising out of or related to any loss, damage, illness or injury (including death) that may be sustained by my child while participating in or traveling to and from this event.
I agree to accept full responsibility, financially or otherwise, for any damage my child may do to the property of Dreamland at Tank Town, other’s personal property, or vehicles used for transportation. I also agree to accept full responsibility, financially or otherwise, for any injury to others which my child may cause, indemnifying Dreamland at Tank Town LLC and SLK Real Estate LLC for any such injuries.
I agree that in exchange for my child using the facilities at Dreamland Dripping Springs, Dreamland at Tank Town LLC, and SLK Real Estate LLC may record and retain my child’s image and otherwise exploit and use my child’s likeness in any media throughout the world for any reason without compensation to me or my child or further approval by me or my child, and I waive any claims related to the use of such likeness or image.
I acknowledge that an inherent risk of exposure to COVID-19 or other illness exists in any public place where people are present.
I understand parts of Dreamland Dripping Springs are still under construction. Any use of its amenities is entirely at my child’s own risk.
I declare that I have read, understood and agree to be bound by the contents of this WAIVER OF LIABILITY in its entirety.
