Damage Deposit Authorization Form

I hereby authorize the use of my credit card as a damage deposit in the event that any damage occurs during the room rental booked at Chappelle Gardens Residents Association on the date listed below.

This credit card will be securely stored for the duration of the rental and will not be charged or pre-authorized unless damage is identified. The card information will be permanently destroyed once a post-event inspection confirms the facility is in satisfactory condition.

Chappelle Gardens Residents Association reserves the right to charge the full cost of repair or replacement for any damages incurred during the rental. While typical charges may be up to $500, if actual damages exceed this amount, the full cost will be charged to the cardholder.

If the cost to repair or replace any individual item—or group of items (e.g., multiple chairs)—exceeds $1,000, the General Manager will obtain three independent quotes to ensure fair market pricing.

If I am not the individual who booked the rental, I acknowledge that I am voluntarily assuming financial responsibility on behalf of the renter listed below and that I have their full knowledge and consent to provide this authorization.

Privacy Statement

All credit card information will be kept strictly confidential and used solely for the purpose of recovering costs related to property damage. Once it is confirmed that no damage occurred, all information will be securely destroyed.
