Thank you for choosing Chappelle Gardens PD Day Camps for your child! The CGRA is a non-profit organization that is responsible for the maintenance of community features, including entry ways, parks, and for running programs and events all year round for all members to enjoy. Please ensure that you read the following information before camp begins. If you have any questions or concerns, please do not hesitate to contact us. If any of your family contact information or camper personal/medical information changes prior to the start of camp, it is imperative that you notify the office.

Cancellation Policy
Refunds and cancellations can be requested up to 2 weeks prior to the day camp start date with a non-refundable administration fee of $15.00. All requests must be made in writing by contacting the Events Coordinator at events@chappellegardensra.ca. No refunds will be issued for programs cancelled less than two weeks prior to the camp start date unless for medical reasons and documentation is provided.

Photo Release
The Chappelle Gardens Residents Association staff take a variety of photos and or video of camp activities. These photos and/ or videos may be used on our social media, website or used for promotional purposes (no names will be included).
I give permission to allow photos or videos of my child to be taken during camp activities. I further give permission to Chappelle Gardens Residents Association the right to any such photos or video to promote camp the experience. *If photos are prohibited we ask that you discuss this with your child before hand so they understand they cannot be photographed.

First Aid Authorization
I confirm the Participant’s medical conditions and information stated on this application is complete and correct. I give permission to the Chappelle Gardens Residents Association Camp staff to (1) provide appropriate first aid for minor injuries; and (2) seek further treatment from emergency services and hospitals if the situation warrants. I certify that I have completed all sections of this form and accept full responsibility for any errors or omissions. The Participant has permission to take part in all program activities except as noted above. I understand the information on this form will be shared on a “need to know” basis with Chappelle Gardens Residents Association Camp staff. I fully understand that the Participant is to abide by all rules governing personal conduct during all activities. Any violation of these rules may result in the Participant being sent home. All medical incidents and severe behavioral incidents will include an incident report form to be sign by the parent or guardian upon pickup.

Sunscreen and Bug Spray
When given permission, Chappelle Gardens Camp Staff will assist in applying sunscreen and/or bug spray to bare surfaces including face, ears, bare shoulders, arms, legs and feet. All sunscreen and insect repellent will be provided by the parent/guardian in the original container with a valid expiration date, where applicable, labeled clearly with the participant's name. Sunscreen and bug spray will be reapplied during the day as needed.

Late Pick-Up Policy
We understand that, at times, lateness to pick up children can be an unforeseeable circumstance. Therefore, the first late instance will not be charged. However, subsequent lateness will result in additional fees.

Sign In and Out Authorization
During morning drop-off, parents/guardians MUST sign their campers in and out. The camp has already received a list of “Authorized Persons” when you completed the registration form; only those listed as “Authorized Persons” will be permitted to drop-off and pick-up the child. If you need to remove or add a person to the authorized list, please notify the office immediately at 780-705-1815.
In addition, individuals must present photo identification the first time that they are picking up the child. Failure to do so will result in the child not being released until the office has contacted the primary contact to receive authorization.
Please remember to bring photo ID the first time that you pick up your child. If an authorized person is listed, they must present photo ID if it is the first time that they come. Chappelle Gardens prides itself on safety policies and procedures. Please do not ask our camp staff to allow you to pick up your child without showing them your ID or having previously shown it.

Walk Home Alone Authorization
For some families, the ability for their child to walk to and from camp is a sign of independence and maturity. In order to ensure our campers safety, we require consent from parent/guardian confirming their camper can walk home alone. Camp leaders will only allow campers over the age of 8 to walk home alone if prior consent has been received. If you click no on the below box, your child will be required to be picked up by one of the authorized individuals stated lower in this waiver. Any child with authorization to walk home alone will NOT be permitted to leave the CGRA before 4pm.
I consent to my child being allow to walk home from camp on their own after the programming is completed. I ensure that my child understands the safety aspects of walking home on their own. I understand that my child will be walking home unsupervised and it is the responsibility of the parent/guardian, and not Chappelle Gardens camp leaders, or the Association, once my child has left the Social House building.

Snacks and Treats
At the end of the camp or during certain activities, the Chappelle Garden Residents Association Camp Staff may provide some peanut-free snacks or treats to the campers (taxing into consideration all allergies and dietary restrictions).
