Welcome to Brew-&-Play Cafè! We’re so glad you’re here. Please review and sign this waiver so everyone can enjoy a safe and fun experience. Your participation in any activity at our facility acknowledges that you understand and accept the following terms.
Assumption of Risk & Liability Waiver:
I understand that participation in play activities involves inherent risks, including but not limited to falls, collisions, and other incidents that could result in injury. I voluntarily assume all risks associated with my child’s participation at Brew-&-Play Cafè, LLC. I release Brew-&-Play Cafè, LLC, its owners, employees, and affiliates from any and all claims, demands, or causes of action related to injuries or property damage occurring while on the premises.
Health & Safety Acknowledgment:
I certify that my child(ren) are in good health and able to participate in play activities. I agree to always supervise my child(ren) or designate a responsible adult to do so. I will follow all facility rules and guidelines for safety and cleanliness.
Photo & Video Release:
I give Brew-&-Play Cafè, LLC permission to use photos or videos taken during visits for social media, marketing, or promotional purposes. I understand that no personal information will be shared and that this consent may be revoked in writing at any time.
Acknowledgment & Signature:
I have read and understood this waiver and agree to its terms voluntarily.
