Release and Waiver of Liability
BLUE ZOO BATON ROUGE

By signing this release and waiver of liability (the “Release”), you do so in favor of Blue Zoo, Inc., its directors, officers, administrators, employees, and agents (collectively referred to herein as “Blue Zoo”) and give up the right to sue Blue Zoo for any injury or damages, of whatever kind or nature, that may arise from or relate to your participation in the programs and activities offered by Blue Zoo (the “Blue Zoo Activities”).

I, [FIRST NAME] [LAST NAME], hereby sign this Release on behalf of myself, my personal representatives, heirs, next of kin and anyone who obtains rights from me, and on behalf of my child(ren).

I desire to participate in the Blue Zoo Activities and understand that such participation may involve risk of bodily harm, serious injury, death, or personal property damage or loss. For this reason, I do hereby freely, voluntarily, and without duress execute this Release under the following terms:

1. Waiver and Release. I do hereby release and forever discharge and hold harmless Blue Zoo and its successors and assigns from any and all fault, negligence, liabilities, costs, claims, actions, demands, penalties, losses, lawsuits, or damages of whatever kind or nature, either in law or in equity, which arise in any way (or may hereafter arise in any way) from my participation in any Blue Zoo Activities.
   
   
2. Medical Treatment. I do hereby release and forever discharge Blue Zoo from any claim whatsoever which arises or may hereafter arise on account of any first aid, treatment, or service rendered in connection with my participation in any Blue Zoo Activities.
   
   
3. Assumption of Risk. I do hereby expressly and specifically acknowledge and assume the risks inherent in the Blue Zoo Activities and release Blue Zoo from all liability for harm, injury, illness, death, or damage to personal property that arises from or relates in any way to my participation in the BlueZoo Activities.
   
   
4. Photographic Release. I do hereby consent to the use of my likeness in print, photographic, digital, electronic, or social media by Blue Zoo in connection with Blue Zoo’s marketing, advertising, and promotion of the Blue Zoo Activities and the Blue Zoo brand.
   
   
5. Scope. I expressly agree that this Release is intended to be as broad and inclusive as permitted by the laws of [insert state] and that this Release shall be governed by and interpreted in accordance with the laws of the State of [insert state]. I agree that in the event any clause or provision of this Release shall be held to be invalid by any court of competent jurisdiction, the invalidity of such clause or provision shall not otherwise affect the remaining provisions of this

   Release, which shall continue to be enforceable.

6. Allergy warning: the food fed to the animals may contain shellfish, seafood, peanuts and other food allergies. Animals, their dander, feathers, saliva, animal feed or bedding may cause allergic reactions in some people.

    
   
   

I have read and fully understand this Release and that I am assuming all risks inherent in participation in the BlueZoo Activities. I voluntarily sign my name, and thereby state my acceptance of the above provisions.
