Welcome from All Star Liveaboards and Aqua Cat. We look forward to having you onboard for an amazing vacation. In order to make the most out of your vacation we want to get your paperwork finished before you arrive.
You will need to sign the Liability Release, the Shark Dive Waiver and then complete a standard diving medical form. We are required to have the Liability Release and Medical Form properly completed before any in-water activities. That is to say, if you do not complete these forms you will not be allowed to go diving or snorkeling.
Please pay special note to the Medical:
- This is the same medical you are required to fill out before any training with any agency. If you have any questions, don't hesitate to ask your Dive Shop, Instructor, or All Star Liveaboard.
- If you answer YES to questions 3, 5 or 10, OR to any of the questions on page 2, please sign it, date it AND take all three pages of this form (Participant Questionnaire and the
Physician’s Evaluation Form) to your physician for a medical evaluation. Participation in diving now requires a Physician's approval.
- If you need to have a doctor signature, no problem, just fill out the form and at the end we will email you your completed and signed copy for you to take you a doctor.
- If you are required to get a doctor's signature on your Medical Form, a doctor's note is not sufficient. A Physician will need to sign the printed and completed Medical Form provided.
I understand that I cannot go diving without these forms completed.
