LIABILITY WAIVER & RELEASE OF RISK: PUBLIC SKATING
Event: American Pond Hockey Public Skating
Date: February 6–8, 20261.
1.) Acknowledgment of Risk,
the undersigned, acknowledge that ice skating is an inherently dangerous activity. Risks include, but are not limited to, falls, collisions with other skaters or structures, and injuries from cold weather or uneven ice surfaces. I voluntarily assume all risks of personal injury, death, or property damage arising from my participation.
2. Safety Requirements (Mandatory)
By signing this waiver, I agree to the following safety protocols:Helmet Requirement: ALL skaters, regardless of age or skill level, are strictly required to wear a safety-certified helmet while on the ice.Minor Supervision: All minors (under the age of 18) must be supervised by a parent or legal guardian at all times while on the premises. American Pond Hockey does not provide childcare or "drop-off" supervision.Code of Conduct: I agree to follow the instructions of rink monitors and staff. Reckless skating, horseplay, or skating against the flow of traffic is prohibited.
3. Release and IndemnificationI hereby release, waive, and discharge American Pond Hockey, its organizers, sponsors, officials, and the venue owners from any and all liability, claims, or causes of action arising out of my (or my child’s) participation in public skating. I further agree to indemnify and hold harmless the aforementioned parties from any claims brought by third parties due to my actions.
4. Medical AuthorizationIn the event of an emergency, I authorize staff to secure medical treatment for myself or the minor(s) listed below if I am unable to provide consent, with the understanding that I am responsible for all associated costs.
