STUDENT/PARTICIPATION WAIVER FOR ABRHS PROJECT GRADUATION PARTY
This Agreement ("Agreement") is made effective between the undersigned, hereinafter referred to as "Parent/Guardian," and Project Graduation and Acton Boxborough Regional High School (ABRHS), collectively referred to as "Hosts."
GRANT OF PERMISSION FOR ATTENDANCE
I, as the parent/guardian, hereby grant explicit permission for my child to participate in the All Night Graduation Party on June 7, 2024, hosted by Project Graduation and Acton Boxborough Regional High School (ABRHS). In consideration of allowing my child to attend this event, I agree to release Project Graduation and Acton Boxborough Regional High School from any and all liability associated with my child's participation.
ACKNOWLEDGEMENT OF RULES AND REGULATIONS
I acknowledge the rules and regulations outlined for this event and affirm that my child will comply with them. Additionally, I acknowledge that Project Graduation is not a school-sponsored event, and therefore, Acton Boxborough Regional High School assumes no legal liability associated with the Party.
ASSUMPTION OF RISKS
I understand that participation in events or activities carries inherent risks that may lead to physical or emotional injury, such as broken bones, sprains, paralysis, or other bodily harm. I attest that my child is in appropriate physical condition to participate and does not have any existing medical or health conditions that could heighten these risks.
MEDIA RELEASE
I authorize Acton Boxborough volunteer representatives to capture photographs, videos, and/or recordings of my child during this activity.
DISMISSAL POLICY
We expect and insist that all students stay for the entirety of the event. However, if your child is over 18, they have the autonomy to sign themselves out, though we strongly encourage them to remain for the duration.
By signing below, I confirm that I have read, understood, and agree to the terms and conditions outlined in this Agreement on behalf of myself and my child.
